If you are submitting your work by US Mail, print out hard copy of the application and fill in your responses. If you are submitting your work by E-Mail, use the Microsoft Word file to fill in your responses and send as an attachment with your email. Note: If submiting by email, type in your full name and date on the "Signature of Applicant/Date" lines. This will serve as your electronic signature. |
